Symptom & Impact
`emgr -l` shows efixes in `S` (stale) state after a TL update and the kernel fails to boot.
Environment & Reproduction
AIX 7.3 LPAR with several IBM-issued ifixes installed via `emgr -e`.
Root Cause Analysis
Stacked ifixes were not removed before the TL update and now collide with new bos.rte.
Quick Triage
Run `emgr -l` and `emgr -P` to list installed and pending fixes; capture the stale labels.
Step-by-Step Diagnosis
Boot from the alternate disk (alt_rootvg) and inspect `/var/adm/ras/emgr.log`.

Solution – Primary Fix
Remove the stale ifix: `emgr -r -L

Solution – Alternative Approaches
Boot maintenance mode and run `emgr -X -L
Verification & Acceptance Criteria
`emgr -l` shows only `S`tate `S` cleared and `bosboot` succeeds on the active rootvg.
Rollback Plan
Activate `altinst_rootvg` via `bootlist -m normal hdiskAlt` and reboot to roll back.
Prevention & Hardening
Always run `emgr -l` and remove or reinstall ifixes during TL/SP planning windows.
