Dual-VIOS setup on AIX 7.2 client with one VIOS rebooted or upgraded.
Root Cause Analysis
MPIO path on the rebooted VIOS stays in Failed state after recovery.
Quick Triage
Run lspath on the client and look for Failed entries on vscsiN.
Step-by-Step Diagnosis
Confirm VIOS is up with cluster -list and lsmap -all on the VIOS.

Re-enable the path on the client with chpath -l hdiskN -p vscsiN -s enable.
